Dr. Korngut’s office is a predominantly Fee for Service practice.
Participation with insurance plans is limited. Payment is expected at the initial treatment visit.

We accept checks and all major credit cards. Payment may also be arranged through a third party who can offer a variety of payment plans for you.

Some plans begin at 0% interest while you wait for reimbursement from your insurance company. We will be happy to discuss any questions you may have.

If you have private medical insurance a request for predetermination of benefits can be sent prior to the start of treatment. We can help you with this. Deductibles and co-pay apply. In this way, you will know if your contract covers you for the therapy.

Oral appliances are considered Durable Medical Equipment (DME). The usual insurance CPT code is E0486. You might want to call your carrier.

Please Note: These oral appliances are considered MEDICAL DEVICES and are not typically covered by dental insurance. Your initial consultation, dental x-rays, and models may be covered in part by dental insurance.

Will my insurance cover oral appliance therapy?

Most medical insurances cover oral appliance therapy for the treatment of Obstructive Sleep Apnea (OSA).

How will I know if I’m covered by my insurance?

It would be best to call your insurance company. After coming to our office for an initial visit we can send out for a Predetermination of Benefits to your insurance carrier for you. Their responding letter will let you know what percentage of our fees will be covered by their company.

Why do I need to pay for care at the time of the visit?

If Dr. Korngut is not a provider in your insurance network then we do not have access to your insurance information.
